You must comply with the current ID documentation checks and verify that you have done so by placing a cross in the appropriate verification box to the right of Question 21 on page 2 and to the right of question 37 on page 3 under the sub-heading ‘registered body use only’ You must see original documentation only and it is advisable that you keep copies of all documents seen. You will need to complete and forward with the application form a document that you have complied with the ID guidelines and listing the documents you have seen in support (see below)
You will also have to ask the applicant to read the DBS privacy policy as to how they handle confidential information and they must sign the document below in confirmation the original of which must be forwarded with the application.
Please note that the DBS require that you state the type of workforce the applicant will be working with. You do this by writing on the back page in line one of question 61 (Position applied for) whether the applicant will be working with adult workforce or child workforce or (if both) adult and child workforce or ‘other’. In line two of question 61 you must state the position applied for e.g. Care Assistant.
If you need a DBS ADULT FIRST in addition to the main disclosure please complete the DBS adult first request form (See list of forms below). Phil will make the application at the appropriate time and e-mail you the result as soon as it is known.
If your application is for a volunteer please also complete the form below entitled ‘Volunteer Applications’ and send with your application.
When all documents are complete forward the application together with necessary continuation forms plus the DBS Adult First Request if needed, The ID confirmation form and applicants confirmation that they have seen the DBS privacy policy form along with the fees due to Phil Roseman, SECHA Administrator, 18, Hullbridge Road, Rayleigh, Essex, SS6 9NZ.
Please note that SECHA cannot accept cheques from applicants personal accounts, only cheques drawn on the homes own account.
The DBS Disclosure will be sent only to the applicant direct by the DBS and you must ask the applicant to show you the original when they receive it.
